THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Our Commitment
The NueSeense Aesthetic Center is required by the Health Insurance Portability and Accountability Act ("HIPAA") to maintain the privacy of your protected health information ("PHI"), to provide you with this Notice of our legal duties and privacy practices, and to follow the terms of the Notice currently in effect.
Uses and Disclosures Without Authorization
We may use and disclose your PHI without your written authorization for:
- Treatment — to coordinate or manage your care with other providers.
- Payment — to bill and collect for services rendered.
- Health-care operations — quality assessment, credentialing, training, audits, and business management.
- As required by law — including public-health reporting, law enforcement requests, and judicial proceedings.
Uses That Require Your Written Authorization
- Most uses of psychotherapy notes.
- Marketing communications (other than face-to-face) and the sale of PHI.
- Use of "before-and-after" photographs for promotional purposes.
You may revoke an authorization in writing at any time, except to the extent we have already acted on it.
Your Rights
- Inspect and copy your medical and billing records.
- Request amendments to information you believe is incorrect or incomplete.
- Receive an accounting of certain disclosures made in the prior six years.
- Request restrictions on specific uses or disclosures (we will accommodate reasonable requests).
- Request confidential communications at an alternate address or phone number.
- Receive a paper copy of this Notice at any time, even if you agreed to receive it electronically.
- Be notified following any breach of your unsecured PHI.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer at thenueseense@gmail.com or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
Changes to This Notice
We reserve the right to change this Notice and to make the revised version effective for all PHI we maintain. The current Notice will always be posted in our facility and on this website.
